Provided:

Immediate response to referrals by our trained office staff
from 9am - 5pm, Monday through Friday. Our staff will answer the phone personally
during these business hours, plus clients can use our live answering
service after regular business hours to reach the "on-call"
clinician regarding urgent issues at 243-2169.
- Urgent appointments are booked within 24 hours weekdays. Routine
appointments are available within 5 working days.
- Most Insurances are accepted and billed on behalf of the
patient.
- Convenient Capital District New York locations in Guilderland,
Schodack/East Greenbush, and Clifton Park.
- Evening and daytime appointments available as well as limited weekend
hours.
- Follow Up by our staff to the referring doctor is available
only with permission.
Appointment
and Support Hours:

Appointment Hours: Depending on the therapist, appointment
times are available from early in the morning through late evening,
Monday through Friday, and there is limited availability on weekends.

Office Support Hours: Our trained support staff is available
Monday through Friday from 9 am - 5 pm to accept referrals, make
or cancel appointments, and answer any questions you might have.

After-hours

For routine issues, you can leave a message on our
confidential voice mail system at any time that the office
is closed. Each therapist has a confidential message box.
Call the main

therapist's extension number is listed by his/her name on
the staff page.

For urgent issues or cancellations, call our answering
service at (518) 243-2169 Our answering service will
contact the "on-call" therapist to provide you with
assistance should an urgent issue arise during non-business
hours. Please understand that although an attempt will be
made to reach your own therapist, if that individual is not
available, then the therapist who is "on-call" for
that week will respond. This phone number is for use regarding
urgent issues and next day cancellations only.

If you are faced with a life threatening emergency, please
call 911 or go to the nearest emergency room for immediate
assistance.

Our Main Office is located at 2280 Western Ave., Guilderland - just
West of the Guilderland Public Library and 20 Mall.

The East Greenbush Office is located on Pond View Road in Schodack,
just off Interstate 90 at the Miller Road exit #10.

The Clifton Park Office is located at 634 Plank Road, Clifton Park, NY
12065. From the Northway take a left on Route 146;
go to the first street and turn right onto Plank Road it is
about 1 mile on your left across from Curves. We are on
the second floor. First suite on the right.
From Route 146 east: pass Shenendehowa High School,
go to Plank Road. It is across from the Northway entrance.
Follow it about 1 mile. We are on the left. Go in
back door second floor, first room on the right.
Payment
Methods and Insurances:

We accept cash, checks, and credit cards. Payment
is due at the time of the visit.

Assessment and therapy services are billed by the "session"
(45-50 minute session) at the following rates:

Assessment and
Short Term Therapy at KPA are typically covered by most insurance carriers.

Life Coaching is not reimbursed
by health insurance companies, because, by definition, the
client is not suffering from any emotional problems that need
to be treated. The cost of coaching is set by the individual
coach and depends on the frequency of services and whether
you are meeting face to face or over the phone or internet.
A typical coaching package would be $350 pre-paid per month
for four 30-minute phone consultations per month and two
10-minute spot coaching check ups.

The Groups on Demand program
has a set fee of $35 per session. Other group programs are
priced as advertised. Most major insurances are accepted for
therapy groups.

Miscellaneous Charges: Check
with your therapist or the office about fees for reports, photocopying,
etc.
Office
Policies:

Confidentiality: Client contact with all KPA
staff is strictly confidential, thus insuring that
therapy may be as useful as possible. There are certain exceptions
however of which you should be aware.

If you are using health insurance, your insurance company
will know of your visits and your diagnosis. Managed care
companies generally require also that they be informed of
other details of your situation so that they can determine
whether the services are medically necessary.

If you are assessed to be suicidal and/or homicidal then
steps will be taken to insure your safety and/or the safety
of others, even though that might break confidentiality. Similarly,
if there is child abuse occurring, all mental health workers
are ethically and legally mandated to report the incident(s)
to Child Protective Services. Typically these situations are
discussed with the patient before any action is taken.

Assessments and therapy that are requested/mandated by an
outside agency (e.g., family court, a school district) generally
require that session information be shared with the mandating
agency, thereby reducing confidentiality.

If you have questions or concerns about the extent of
confidentiality in your individual situation, please be sure
to discuss them with your therapist. This is especially
important for couples and family therapy, to insure that
everyone understands and is comfortable with the amount of
information to be shared.

Information about your contact with KPA can always be
released to anyone you choose, with your written consent.
Patients often request that information be released to
their primary care physician, teachers, employers, etc. for
various reasons. If the sharing of information will be extensive,
there may be additional charges to the patient for the service.
This can be discussed with your therapist.

No Show Policy: Appointments that are not canceled
at least 24 hours beforehand are charged full fee; insurance companies will not cover
any portion of a missed appointment, making the entire cost (not just the usual copay) the
responsibility of the client. If you are going to miss an appointment, please contact the therapist or
receptionist (or- if after hours- the answering service at 243-2169) immediately. If there are
extenuating circumstances (last minute illness, dangerous weather), we will generally waive the late
fee upon explanation.

Our front desk receptionists are specially trained to respond
to patients' concerns in a prompt and professional manner. They
schedule patient appointments with the therapists, provide appointment
reminder calls and answers to most questions.
Marie (x208) the new patient coordinator;
takes the initial referral information and helps
assign them to the best therapist for their unique needs.

Assistant to Dr.'s Doberman and Mooney: Mary Kay (x204)
Mary Kay provides assistance for patients of Dr. Mooney
and Dr. Doberman including, scheduling appointments and helping patients with any additional questions
they might have.

Each therapist has his/her own extension number, which is listed
in their biography, thus enabling you to leave him or her a confidential
voice mail message any time, day or night. Please understand however,
that therapists typically do not interrupt sessions for phone
calls.

can put you in touch with the on-call therapist. More information
