New Patient Paperwork

The Consent to Treatment and Telehealth Consent forms are due immediately following the booking of your appointment. The Clinical Questionnaire is due within 48 hours.

We offer patients two methods for completing their required new patient documents. You can easily complete the forms digitally online from your computer or tablet or you can download and print forms to complete by hand.

Choice #1 - Digital Forms

Step 1. Fill out the Consent to Treat Form Immediately

Step 2. Complete the New Patient Clinical Questionnaire ASAP, no Later than 48 Hours Before Your Scheduled Appointment.

Choice #2 - Handwritten Downloadable Forms

Step 1. Download the Consent to Treat Form and the New Patient Clinical Questionnaire.

Step 2. Complete and Return to Karner Psychological Associates ASAP, no Later than 48 Hours Before Your Scheduled Appointment.

How to submit these forms for an IN-PERSON/IN OFFICE VISIT:

If you are being seen in-person, please bring the completed forms with you. Please complete and give to your therapist at the time of the visit.

If you forget to do your paperwork beforehand, there are extra blank new patient forms available in the waiting room.  We ask that you come 15 minutes early to complete the paperwork.

How to submit these forms for a TELEHEALTH VISIT:

Telehealth patients must return these forms 48 hours before their appointment!

 

Options for returning handwritten forms:

  • Option #1 – Fax the forms to 518-456-6512

  • Option #2 – Drop them off at our Main Office at 2280 Western Avenue, Guilderland, NY 12084.

  • Option #3 – Mail them to Karner Psychological Associates at the Guilderland Office.

No Show Policy

Appointments that are not cancelled at least 24 hours beforehand will be subject to a fee of $50! 

Insurance companies will not cover any portion of a missed appointment, making the entire cost the responsibility of the client. Our therapists are not paid if you do not show for your appointment or fail to cancel 24 hours ahead which allows us to schedule another client. Therefore, be careful to schedule appointments only at times when you are sure that you can attend. If you are going to miss an appointment, please contact the therapist or receptionist (or, if after hours, the answering service at (518)243-2169) immediately. If there are reasonable extenuating circumstances (last minute illness, dangerous weather),  you may expect that this fee will be waived.
